Job Summary:
The Social Worker assesses the social needs of the assigned patients/families coordinating a plan of care that meets the unique needs of the patient and his/her family. Collaborates and participates in the disciplinary planning process and coordinates pre/post-hospital care, and effective use of community resources to integrate care, improve quality of care, and decrease cost and utilization.Core Responsibilities and Essential Functions:
Coordination of Care:
Conducts an appropriate level of psychosocial assessments on all patients referred for social work services. Process and coordinate long-term placement/durable medical equipment and supplies/utilize community resourcesConsultation/Communication:
Collaborate with the patients, families, caregivers, legal guardians, and healthcare team of factors that may potentially inhibit their understanding of the patient's pre/post-hospital care.Documentation:
Provides communication and substantiation of services rendered through clear, concise, and timely documentation in the patient's electronic medical records.Self-Management:
Effectively prioritizes referrals. Produces acceptable work volume and effectively meets established deadlines. Works effectively with the direction provided independently and consistently.Education:
Contributes to an environment that is conducive to the clinical education of students, as appropriate. Provides education to physicians, staff, patients, and families regarding coordination of care and related issues. Maintains job-related knowledgeable skills, pursues self-development opportunities. Facilitates and maintains social work interns' school requirements- when needed.
Required Minimum Education:
Masters Social Work Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated. BLS- Basic Life Support or ARC-BLS
- Amer Red Cross Basic Life Support or BLS-I
- Basic Life Support
- Instructor Additional License(s) and Certification(s):
Required Minimum Experience:
Minimum 2 years experience in healthcare or hospital setting. Required Minimum 2 years Post-Master's degree experience.Preferred Required Minimum Skills:
Excellent assessment skills- Strong organizational skills
