At Bridgeway Supportive Living, we are dedicated to providing exceptional care and enriching lives for our residents. We are seeking a creative, compassionate, and dynamic Memory Care Coordinator to join our team and oversee a vibrant activity program for our residents.
Who We Are Looking For:
We're looking for a compassionate, organized, and resourceful Memory Care Coordinator to join our team. The employee would be expected to work directly with our residents and their families, helping them navigate long-term care while promoting their emotional and social well-being. Your ability to listen, understand, and advocate for their needs will be crucial in creating a supportive and caring environment for everyone.
Job Overview:
The Memory Care Coordinator will be responsible for planning, coordinating, and overseeing various aspects of the activity program. This role requires creativity, adaptability, and a strong commitment to resident engagement. This employee will conduct assessments to get to know each resident's social, emotional, and psychological needs. This includes meeting new residents, updating care plans as needs change, and working with our interdisciplinary team to ensure every resident's experience here feels personalized and supportive.
Key Responsibilities:
  • Design and implement engaging and enriching activities that address the physical, mental, and emotional needs of residents.
  • Ensure the safety of all participants during activities.
  • Provide direct care and supervision to residents during activities including offering assistance to promote active participation.
  • Collaborate with direct care staff to foster an inclusive, supportive, and socially engaging environment for residents.
  • Plan and oversee daily activity schedules, ensuring programming is consistently carried out seven days a week, including weekends and holidays.
  • Stay on call to address any programmatic needs, ensuring seamless activity execution at all times.
  • Maintain accurate records of resident participation, progress, and feedback to continually improve programming.
  • Accurate documentation is key.
The employee would be expected to maintain up-to-date records for each resident, ensuring we meet all local, state, and federal regulations.
Qualifications:
Previous experience in activity planning and memory care in a long-term care setting. Elevated communication skills with the residents as well as the families.
