Care Coordinators - Full-Time Opportunities Available! Works independently while exercising sound judgment and discretion to assess the needs of individuals and families. Develops and implements comprehensive care plans to support clients' overall well-being. May work in office settings, community locations, or conduct home visits. Conducts intake assessments for new clients within required timeframes. Provides ongoing case management services by educating clients and their families on available programs, benefits, and community resources, and assists with accessing those services. Ensures timely and accurate documentation. Responds to all client and partner inquiries within 24 hours. Develops clear, detailed, and effective care plans tailored to each client's unique needs, ensuring compliance with agency policies, funding requirements, and regulatory standards. Coordinates services by making appropriate referrals, monitoring client progress, and adjusting care plans as needed. Collaborates with internal teams and external partners while maintaining proper documentation and completing required reporting. Utilizes and contributes to community resource networks to enhance service delivery and support for clients. Maintains accurate and up-to-date client records in accordance with agency policies, professional standards, and regulatory requirements. Participates in team meetings, trainings, and community outreach efforts to support program growth and client engagement.

Qualifications:

Bachelor's or Master's degree in a Human Services or related field (Required) Minimum of 1 year of case management or care coordination experience (Required) Reliable transportation and valid Driver's License (Required)
