Job Description

The Care Coordinator for Pathways CCSO Services supports youth and families through trauma-informed, individualized care coordination. This role involves building relationships, leading care team meetings, coordinating services, and ensuring each family's voice drives the care plan. Description Essential Job Duties and Responsibilities Provide client- and family-driven care in alignment with the Pathways Care Model, maintaining appropriate caseloads. Coordinate and facilitate services, including Child and Family Team (CFT) meetings, to support safety, crisis prevention, and individualized needs. Engage families and supports through in-home visits, phone, email, and other methods of contact. Collaborate with Managed Care Organizations, providers, and the mobile crisis team to ensure continuity and access to care. Complete accurate and timely documentation in compliance with organizational, state, and federal requirements.
Qualifications:
Requires current standing as a Mental Health Professional credential in the State of Illinois Bachelor's degree in a behavioral/social science field from an accredited college or university preferred Prior experience providing direct services in case management or care coordination services in accordance with State of Illinois Medicaid criteria preferred. Familiarity and willingness to learn and work with computer-based systems Excellent written and verbal communication skills Bi-lingual Spanish/English preferred
Compensation and Benefits:
Pay Range:
$21.29-$31.93 per hour
