POSITION SUMMARY
The Case Management (CM) Assistant works collaboratively with Case Management and the Treatment Team to support timely patient progression to the appropriate level of post-hospital care throughout the patient's hospital stay. Early in the patient's hospital stay, the CM Assistant will interview the patient, caregiver, and/or decision-maker to document the patient's baseline functioning in the pre-hospital environment and complete the discharge evaluation as required by Centers for Medicare & Medicaid Services (CMS). Through the patient's hospital stay, the CM Assistant interacts directly with patients, caregivers, and/or decision-makers and gathers pertinent patient information for continued care transition needs while providing excellent patient experience. Ensures all communication is appropriate, accurate, timely, and consistent with the patient's plan of care. Qualifications
REQUIRED QUALIFICATIONS
Education:
Associate's degree with one year of healthcare experience OR Bachelor's in social work (BSW) with or without licensure OR Bachelor's degree in a healthcare or social services related field
Other Skills/ Knowledge:
Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem-solving skills, with the ability to be detail oriented. Demonstrated proficiency in communicating sensitive complex information to persons in a hospital environment. Demonstrated ability to work independently and execute their own workload. Critical thinking ability necessary to prioritize workload to ensure tasks are completed in a timely manner and within established timeframe. Interpersonal skills necessary to facilitate interpersonal communication with team members, department staff, and external personnel, interact with patients/families, and perform other activities requiring courtesy and tact. PREFERRED QUALIFICATIONS
Education:
Bachelor's degree in social work (BSW)
Experience:
Social work internship in a healthcare setting Home care, post-acute care, discharge planning and care coordination, and/or pertinent experience.
Other Skills/ Knowledge:
Working knowledge of computer software applications and electronic health records. Familiarity with medical terminology. Proven ability to think critically, work independently, and prioritize own workload to meet deadlines. OSF HealthCare is an Equal Opportunity Employer.
