FOR CARE COORDINATOR
Each employee will further the goals and objectives of Senior Services of Central Illinois, Inc. and will comply with policies and guidelines established by the Board of Directors and any/all funding agencies. The Choices for Care Coordinator will cooperate fully with all other projects of Senior Services of Central Illinois, Inc. in order to provide services that prevent unnecessary or premature institutionalization of older persons and to foster their independence, dignity and an improved quality of life.
Duties:
1. Complete Choices for Care assessments in the hospital and nursing facilities within the required timeframes throughout Sangamon, Menard, Logan and Mason counties. 2. Complete all associated paperwork within a maximum of 5 days. 3. Offer the participant alternatives to Long-term Care placement. 4. Provide follow-up after the participant has been placed in a LTC facility to determine if it's appropriate for them to go home with community services. 5. Provide Information and Assistance services for participants. 6. Obtain and maintain State certification for Comprehensive Care Coordination. 7. Complete annual in-service hours as required by IDOA. 8. Participate in SSCI Agency and CCU Departmental programs and committees as requested by the Department Director, and attend agency and staff meetings as required. 9. Perform after-hour screens while on rotation or as needed to fulfill required deadlines. 10. Perform other duties as assigned.
Supervision:
Reports directly to the
CCU Director Qualification Requirements and Experience:
Bachelor's Degree in Social Services or Certification as a Registered Nurse and. 2 years' experience in social services settings. Experience in Gerontology is desirable. Knowledge of services available to older adults is desirable. A valid driver's license as well as a reliable vehicle is required to fulfill the job duties of this position. Good interpersonal skills, communication skills, and the ability to work both independently and as a team are required. This position requires the ability to communicate effectively in person, by phone, and via electronic means. Of highest consideration is empathy.
Physical Demands/Work Environment:
Mobility & Stamina:
Ability to sit or stand for extended periods during client meetings, conferences, or office work.
Fieldwork:
Ability to operate a motor vehicle to travel to hospitals and nursing facilities. Ability to walk moderate distances, navigate elevators and stairs, lift and carry items such as a laptop, handheld scanner, etc. weighing up to 25 pounds.
Sensory & Communication:
Basic visual and auditory acuity are required to conduct assessments, recognize non-verbal cues, and interview patients effectively.
Office Work:
ability to perform duties in a standard office environment with frequent use of a computer and other office equipment. Ability to sit for prolonged periods of time with occasional walking, standing, bending, or reaching. Manual dexterity is required for frequent use of a keyboard and other standard office tools.
