Job Description

POSITION SUMMARY
Position Summary:
The Transitions of Care Quality Coordinator facilitates safe, timely transitions of patients from the hospital to home by coordinating and scheduling all post-discharge follow-up appointments prior to discharge. This role serves as a clinical expert in transitional care workflows and EHR documentation, while collaborating with the multidisciplinary team to continuously improve discharge planning processes and outcomes.
POSITION QUALIFICATIONS
Minimum Education Associate Degree in Nursing (ADN) Minimum Experience Minimum of 2 years' experience in a healthcare setting Required Registration/License/Certification Current, unrestricted Registered Nurse (RN) license in the state of practice
