Heritage Grove Estates (ALF) is seeking a compassionate and organized Resident Care Coordinator to join our team! As a Resident Care Coordinator, you will play a vital role in ensuring the highest level of care and comfort for our residents. Coordinate Resident Care:
Collaborate with healthcare teams to develop and implement personalized care plans that meet the unique needs of each resident Assist withDaily Living Activities:
Provide assistance with daily living activities such as bathing, dressing, and grooming to support residents' independenceMonitor Resident Health:
Conduct regular wellness checks and report any changes or concerns to the healthcare teamMaintain Accurate Records:
Document resident care plans, progress notes, and vital signs in accordance with company policies and regulations Communicate withFamilies:
Keep families informed of their loved ones' progress and any changes or concerns Participate inQuality Improvement Initiatives:
Contribute to quality improvement initiatives to ensure the highest level of care and comfort for residentsRequirements:
