Job Description
Summary The Intensive Care Coordinator (ICC) is responsible for coordinating all services and support identified in the youth's Individual Care Plan (ICP). They are responsible to the Senior Care Coordinator, CSA Assistant Program Director and the CSA Program Director. Job Responsibilities & Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions. Completes initial and ongoing training on the use of Wraparound and how it applies to the work that is done with families. Completes intakes with families once assigned in alignment with provision of service requirements set by MassHealth. Develops a risk management/safety plan in collaboration with the family and collaterals. Completes a comprehensive, home-based assessment inclusive of the CANS Assessment and other tools as determined necessary, in collaboration with the family and other providers, which occurs in the youth's home or another location. In collaboration with the family and the Family Partner (when applicable), identifies who the family would like to be part of the Care Planning Team (CPT). Develops and implements a youth
- and family-centered Individual Care Plan (ICP) in collaboration with the family and other collaterals.
