Lowell Salary Range:
$22.00- $28.
What you will do:
Provide care planning and coordination of services for eligible youth under the age of 21 using the Wraparound model consistent with System of Care philosophy. Facilitate Care Planning Team meetings and coordinate multiple services/ supports. Help caregivers learn effective advocacy skills. Collaborate with all members of the team to ensure continuity in treatment, services, and messages delivered. Attend meetings outside of Care Plan Meetings per request of the parent/guardian and youth. Assist in system navigation regarding school, court, state agencies (DCF, DMH, DDS) along with community resources for youth, young adults, and families.Competencies/Traits of Our Superstars:

Requirements:
Bachelor's Degree in Social Work, Psychology, or related field plus at least 1 year of direct experience working with children, young adults, and/or families. Driving in the community is a requirement of this position; please ensure you have a valid driver's license and access to a reliable vehicle.We will train you in:
