Job Description

The Care Manager (RN, LPN) will serve the North Shore area. Our Care Managers will work to develop strong relationships with clients, families and partners to provide face-to-face, over the phone and telehealth services to identify and address social determinants of health, build up and engage the social support network to help our clients and families navigate through healthcare, transitions, and other services to support them where they live and improve their health and wellbeing.
Responsibilities:
Assess Client Needs Able to evaluate the strengths and needs of primarily older adults. Educate clients and their families/care partners about options and resources. Develop Care Plans Create and manage client-centered plans of care and update them as needed. Facilitate client care plan and recommendation team meetings. Maintain Client Relationships Promptly address any concerns and provide resolutions to provide the best support and care for the client and their family/care partners. Keep Detailed Records Maintain up-to-date client notes. Collaborate with Partners Work closely with community resources and external providers to ensure clients receive the coordinated services and care that is needed and desired. Stay Current with Industry Trends Participate in on-going professional development. Keep pace with industry trends, best practices, and regulations. Provide Updates Participate in regular communication with leadership on assigned clients. Minimum Requirements Bachelor or associate degree in Social Work, Nursing, Healthcare Administration, or a related field. At least 3 years of experience in senior care, home care, or a related healthcare environment. Nurse or OTR/L preferred . Solid understanding of industry standards, emerging trends, and preferably an understanding of SNF and AL regulations. Outstanding communication and interpersonal skills with a knack for problem-solving. Proven ability to handle multiple priorities in a fast-paced setting and ability to reprioritize. Dedication to delivering excellent customer service. Proficiency with Google Workspace and Microsoft Office Suite and ability to adapt to new technologies and software.
Pay:
This role is a pay by visit position and averages $40/hour.
Hours/Schedule:
Typical work schedule is Monday through Friday, 8:30am to 5:00pm, but hours are flexible based on client needs.
