Job Description

JSSA is looking for a full-time Team Coordinator to join our fast paced Hospice team. This position will be the first line resource to the Hospice Clinical Managers for schedules, staffing, and coordination of the patient's hospice care including DME, aides, medical supplies, office supplies and any administrative documents to ensure timely and efficient care delivery using the Homecare Homebase electronic health record system.
Responsibilities:
Central point of contact as one of the primary members of the Hospice Call-Center. Effectively communicates schedule changes, urgent patient needs and issues to patients, families, staff, and other healthcare professionals. Promptly confirms and relays information such as DME requests, aide coverage, and verification of orders received, between patients, families, staff, and service providers. Performs general office duties, and supports the clinical team with any administrative tasks as needed. Create schedules, communicate changes, maintaining patient and staff records in the HCHB electronic health records (EHRs), managing staff availability, and ensuring compliance with care plans and agency policies.
Qualifications:
Associate degree. Bachelors' level preferred. 3-5 years' experience performing phone related work under pressure in the healthcare field. Demonstrated organizational and time management skills. Able to multitask and work in a fast-paced environment. Demonstrated flexibility, adaptability, and outstanding customer service skills. Highly proficient with MS Office 365 applications, Homecare Homebase Electronic Medical Record, and other databases. Familiarity and comfort with medical terminology. Ability to manage confidential information with sensitivity. Capable of communicating professionally with patients, power of attorneys, legal guardians, primary care physicians and other community/medical providers. Able to work in the office Monday through Friday from 8:30am-5pm.
