Activity Therapy Coordinator

Collaborate with the care team to plan, implement, and evaluate activity care programs Plan and conduct appropriate activity programs for patients Evaluate and document patients' activities, progress and the emotional, social, mental and/or physical condition of patients through the activity. Organize and maintain program materials, supplies, and equipment Communicate with family members regarding participant's status Monitor participants continued eligibility for program Participate in staff meetings and committees as assigned Ability to develop educational programs on a specific topic Perform other duties as assigned Associates degree in a related field or an equivalent combination of education and experience Three years of demonstrated and specific experience working with older adults with mild to moderate dementia and their family/caregivers

PHYSICAL REQUIREMENTS

This position requires frequent standing/walking, as well as lifting, positioning, pushing, and/or transferring of participants. Experience with person-centered activity planning for persons living with dementia. Supporting family caregivers with education, and referrals of community resources. This position is covered under the collective bargaining agreement between the U-M and the United Michigan Medicine Allied Professionals (UMMAP), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.
