This role coordinates individualized treatment plans, keeps care on schedule across levels of care, and ensures clients and their support systems stay informed and engaged from admission through discharge. The ideal candidate is highly organized, comfortable working within a multidisciplinary team, and grounded in a genuine commitment to client care. Key Responsibilities Treatment Planning & Coordination Coordinate the development of individualized treatment plans in collaboration with therapists, case managers, and medical staff. Schedule and track individual, group, and family therapy sessions, medical appointments, and psychiatric evaluations. Attend and contribute to treatment team meetings; document and communicate updates to the plan of care. Monitor client progress against treatment goals and flag concerns to the clinical team promptly. Client & Family Communication Serve as the primary liaison between clients, families, and the clinical team, providing regular, compassionate updates on progress. Facilitate family sessions and communicate program expectations, house rules, and milestones. Respond to client and family questions and concerns, escalating clinical matters to the appropriate provider. Administrative & Compliance Maintain accurate, timely documentation in the electronic medical record (EMR) in accordance with agency and regulatory standards. Coordinate with utilization review/insurance staff on authorizations and continued-stay reviews. Ensure all coordination activities comply with HIPAA and 42 CFR Part 2 confidentiality requirements. Support survey readiness and adherence to accreditation standards (e.g., CARF or The Joint Commission) and state licensing requirements. Discharge & Continuing Care Coordinate discharge planning, including referrals for step-down levels of care, outpatient providers, sober living, and community resources. Assist with aftercare and continuing-care planning to support long-term recovery outcomes. Communicate with referral sources and outside providers to ensure a smooth transition of care. Qualifications Required Bachelor's degree in psychology, social work, counseling, human services, or a related field — or equivalent relevant experience. 1+ years of experience in a behavioral health, substance use treatment, or healthcare coordination setting. Working knowledge of HIPAA and 42 CFR Part 2 confidentiality regulations. Strong written and verbal communication skills, with the ability to work sensitively with clients and families in crisis. Excellent organizational skills and the ability to manage multiple client caseloads simultaneously. Proficiency with electronic medical record (EMR) systems and Microsoft Office. Current CPR/First Aid certification, or willingness to obtain within 30 days of hire. Preferred Familiarity with ASAM levels of care and utilization review processes. Certification or progress toward licensure (e.g.
CAC, LADC, LPC, LCSW
) or equivalent state credential. Experience with behavioral health EMR platforms (Procentive or Ritten) or similar case management software. Bilingual candidates are encouraged to apply. Work Environment & Physical Requirements This role is based on-site at a licensed treatment facility and may require occasional evening, weekend, or on-call availability depending on client census and program needs. Requires the ability to sit, stand, and move throughout the facility for extended periods, and to respond calmly to occasional emotionally charged or crisis situations.
Pay:
$22.00 - $25.00 per hour Expected hours: 80.0 per week
