At Advanced Medical Home Care, we're looking for a licensed Physical Therapist Assistant (PTA) or Certified Occupational Therapy Assistant (COTA) to join our team of Clinical Care Coordinators. Your responsibilities will include:
Coordinating communication between physicians, referral sources, patients, caregivers, and our clinical team. Supporting scheduling and administrative processes that keep patient care running efficiently. Working closely with clinicians to ensure timely, compassionate, and well-coordinated care. Someone Who:
is a licensed COTA or PTA Has strong communication and organizational skills. Enjoys solving problems and managing details. Is compassionate, dependable, and patient-focused. Works well independently while thriving as part of a collaborative team.
Required:
Current Minnesota
PTA/COTA
license in good standing. Knowledge of medical terminology Good judgment and decision-making skills. Strong attention to detail. Technologically proficient and driven by accuracy. Regular attendance and flexibility with scheduling necessary.
