Salary Base salary:
$21.67 - $25.10/hourly The expected compensation range for this position represents a good faith estimate for this position and the specific compensation offered to a candidate may vary based on factors including, but not limited to, the candidate's relevant knowledge, training, skills, work location, experience, and/or FTE. Canvas Health's mental health and substance use disorder providers, clinicians, and staff bring their diverse lived experience to help inform and guide the care they provide to clients. Our integrated programs and services offer coordinated care to people of all ages in their homes, the community, and in our clinics. Canvas Health is a non-profit, Certified Community Behavioral Health Clinic (CCBHC) specializing in serving clients who are on Medicaid. Position Summary Coordinates social services provided to residents of assigned senior and low-income buildings. Services include information and referral, outreach services, health and wellness education and assessment of service needs. Location Stillwater, Oakdale and Forest Lake MN Schedule Part-time, 20 hours per week divided between Monday - Friday Minimum Qualifications 2-year degree (AA) Thirty-six (36) hours of in age-related and disability issues training during the first year of employment Knowledge of the methods, values and principles of social work Knowledge of care management issues with seniors and persons with disabilities Ability to establish and maintain effective working relationships with others including Washington County staff, building managers and other professionals Ability to utilize counseling techniques with seniors and their families to achieve care plan goals Knowledge of pertinent state laws regarding vulnerable adult and some state housing regulations Knowledge of HIPAA regulations Ability to gather data, write reports and maintain clients' files accurately and up to date Working knowledge of computers and computer applications Ability to do public speaking and present information to residents, managers, stakeholders including funders and other housing and social service officials Ability to effectively and positively communicate and function as a team member and ability to work independently Preferred Qualifications Bachelor's degree in social work, Gerontology, Psychology or Counseling preferred; other college degree or comparable work experience may be considered 3 years' clinical experience in community-based counseling Two (2) to three (3) years' experience in social service delivery with senior citizens and/or nonelderly disabled Experience coordinating training/educational opportunities and knowledge of referral resources Canvas Health is an Equal Opportunity Employer committed to creating a diverse workforce.
