Job Description

Care Coordinator / Population Health Coordinator Full-Time | Blue Springs Family Care Blue Springs Family Care is seeking a motivated, detail-oriented Care Coordinator to join our team. This position plays a key role in helping our patients stay healthy by coordinating preventive care, closing care gaps, supporting chronic disease management, and assisting with quality improvement initiatives. The ideal candidate is organized, compassionate, and comfortable communicating with patients by phone while working independently to achieve population health and quality goals. Responsibilities Identify and address care gaps related to preventive screenings, chronic disease management, and quality measures Contact patients to schedule Annual Wellness Visits (AWVs), follow-up appointments, and recommended preventive services Assist with Medicare Annual Wellness Visit workflows and outreach efforts Monitor and track quality metrics and value-based care measures Review patient charts to identify opportunities for preventive care and chronic disease management Coordinate referrals, diagnostic testing, and follow-up care as needed Work closely with providers and clinical staff to ensure patients receive recommended care Educate patients regarding preventive health services and treatment recommendations Document patient interactions accurately within the electronic medical record (EMR) Participate in quality improvement projects and population health initiatives Maintain HIPAA compliance and patient confidentiality Qualifications Experience in a medical office, healthcare coordination, medical assisting, nursing, billing, or a related healthcare field preferred Knowledge of preventive care guidelines, Medicare Annual Wellness Visits, and chronic disease management preferred Familiarity with quality measures such as HEDIS, MIPS, or value-based care programs is a plus Strong communication and patient engagement skills Excellent organizational skills and attention to detail Ability to manage multiple priorities and work independently Experience using electronic medical records (EMR) systems Strong computer and documentation skills What We Offer Full-time, Monday through Friday schedule Competitive pay based on experience Benefits package available Supportive team environment Opportunity to make a meaningful impact on patient health outcomes Stable position within a respected, independently owned family medicine practice At Blue Springs Family Care, we are committed to providing high-quality, patient-centered care.
