Job Description

Citizens Memorial Hospital (CMH) in Bolivar, MO is seeking an organized, compassionate, and service-oriented Emergency Department Care Coordinator to help patients successfully transition from the Emergency Department to ongoing outpatient care. This non-clinical role plays a vital part in improving access to care, reducing avoidable emergency department utilization, and supporting positive patient outcomes through proactive care coordination. CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!

About the department:
Care coordination advances population health by ensuring patients discharged from the ED are successfully connected to primary, specialty, and community-based services to promote the right care at the right time in the right setting.
A typical day includes:
  • Coordinate emergency department follow-up appointments with primary care providers, specialists, and ancillary services.
  • Assist patients in establishing primary care relationships when needed.
  • Facilitate referrals and support continuity of care processes.
  • Collaborate with physicians, nurses, clinic teams, and community partners to ensure seamless care transitions.
  • Manage scheduling, referral tracking, and electronic work queues.
  • Support population health and patient experience initiatives.
Job Requirements:
  • High school diploma or equivalent
  • Minimum one year of healthcare-related experience
  • Strong communication, customer service, and organizational skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Proficiency with electronic medical systems.
