This position will manage a caseload of no more than 25-30 clients. The coordinator will spend one set day each week at our Women's Recovery House. Remaining workdays will rotate between the office, Recuperative Care, and Street Outreach based on client needs and the team calendar. The Community Care Coordinator will also serve as the primary resource for routine client-support questions from Day and Night Shift staff, coordinate the schedules of community service partners, and represent Haven at selected community events. Duties and Responsibilities Manage a caseload of no more than 25-30 clients across multiple Haven programs. Assess each client's needs and develop a client-led care plan with clear goals and next steps. Help clients access health care, benefits, identification, food, clothing, housing, transportation, recovery support, mental health care, legal assistance, and employment resources. Help clients complete applications, gather documents, contact providers, and follow up on referrals. Coordinate health, dental, behavioral health, and community appointments. Work with the nurse on medical needs, care instructions, equipment, medications, and clinical discharge concerns. Support safe housing placement and discharge planning from the beginning of services. Answer routine questions from shift support staff about resources, appointments, transportation, housing, benefits, and client support. Give shift staff clear follow-up tasks, including routine calls, confirmations, reminders, and information gathering. Maintain an accurate resource guide with current phone numbers, hours, eligibility rules, and referral steps. Coordinate community partner schedules, including occupational therapy, art therapy, psychotherapy, and other client services. Confirm partner dates, times, space needs, client participation, and schedule changes. Conduct street outreach and build trusting relationships with people who may need services. Attend selected resource fairs, outreach activities, partner meetings, and community events. Use motivational interviewing and practical support to help clients work toward their goals. Maintain timely and accurate client records, referrals, outcomes, barriers, and next steps. Participate in case review and coordinated care planning with nursing and other Haven team members. Complete discharge documentation and provide clients with clear written plans and warm handoffs. Skills and Qualifications Strong interpersonal skills and the ability to build trust with people from different backgrounds. Compassionate, respectful, and trauma-informed approach to client care. Strong organization, follow-through, and time-management skills. Ability to manage a mobile caseload across several programs and locations. Ability to answer staff questions clearly and help the team identify practical next steps. Knowledge of health care, housing, benefits, behavioral health, recovery, and community resource systems. Strong written and verbal communication skills. Ability to maintain accurate and timely records. Ability to work independently while communicating closely with a team. Experience using motivational interviewing or other person-centered approaches is preferred. Experience serving people affected by homelessness, substance use, mental health needs, or complex health conditions is preferred. Ability to work in office, residential, recuperative-care, community-event, and street-outreach settings. Education and Experience High school diploma or equivalent required. Bachelor's degree in social work, public health, psychology, human services, or a related field preferred. Previous experience in case management, community health work, health care, behavioral health, recovery services, housing, or community outreach preferred. Community Health Worker, Certified Peer Specialist, or other relevant credential is a plus but not required.
