Perform services in accordance with the physician's orders and the established plan of care (POC). Qualifications Must possess a bachelor's degree from a school of social work accredited by the council on social work education. Must be certified or licensed per regulation of the state where they practice. Must have at least one year of social work experience in a health care setting. If employment with a hospice branch of the company began prior to Dec. 2, 2008, may not require supervision by a licensed master of social work. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred Medicare home health or hospice experience is preferred.
