Job Description

The Social Services Coordinator plays a key role in helping patients and families access the resources and support they need to live healthy, safe, and independent lives. As part of an interdisciplinary care team, this position works closely with providers, nurses, behavioral health professionals, pharmacists, and care managers to address social determinants of health and remove barriers to care. Key responsibilities include conducting patient assessments, connecting individuals with community resources, assisting with insurance and financial assistance programs, supporting care transitions, facilitating advance care planning, and empowering patients to achieve their health goals through education and self-management support. The Social Services Coordinator also collaborates with community agencies and healthcare teams to ensure patients receive the appropriate level of care and support. This role is ideal for a compassionate, resourceful professional who is passionate about advocacy, care coordination, and improving the overall well-being of patients and their families.
EDUCATION
Bachelors of Social Work required
LICENSE/CERTIFICATION/REGISTRY
BSW required
