Job Description

Overview We are seeking a dedicated and energetic Resident Care Coordinator to lead and coordinate resident care services within our assisted living community. This vital role involves overseeing daily care operations, developing personalized care plans, and ensuring residents receive compassionate, high-quality support. The ideal candidate will bring a strong background in senior care, nursing, or home and community care, along with excellent management and leadership skills. Responsibilities Coordinate and oversee the delivery of resident care, ensuring all services meet individual needs and preferences. Develop, implement, and regularly update personalized care plans in collaboration with residents, families, and healthcare teams. Supervise caregiving staff, providing guidance, training, and performance evaluations to ensure quality standards are maintained. Monitor medication administration and ensure compliance with safety protocols and regulatory requirements. Collaborate with healthcare professionals to support residents with dementia, Alzheimer's disease, or other memory impairments through specialized memory care programs. Facilitate communication between residents, families, staff, and external providers to promote transparency and satisfaction. Skills Extensive experience in assisted living, nursing homes, or long-term care environments. Proven supervisory and management skills within senior or group home settings. Strong understanding of dementia care, Alzheimer's care, and memory support strategies. Ability to develop comprehensive care plans tailored to individual resident needs. Knowledge of medication administration procedures and safety regulations. Excellent organizational skills with experience managing budgets and operational workflows. Compassionate leadership with a focus on resident-centered care and team development. Familiarity with social work principles, financial acumen related to healthcare operations, and home & community care practices.
