Position Summary The Clinical Care Coordinator position will provide immediate linkage to appropriate levels of care for women identified through comprehensive assessment as requiring enhanced services. Responsibilities will include conducting comprehensive SUD/MH scales, using the state-funded vehicle to assist with transporting and ensuring access to outpatient treatment, recovery support services, community resources, and specialized PPW residential treatment programs, including state-funded inpatient PPW programs. Must possess valid NC driver's license and be able to clear a motor vehicle check in order to transport minors. Minimum Education and Experience Requirements Bachelor's degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board. Management Preferences Eligibility of becoming a Qualified Professional status. History, knowledge, and exposure to social work or counseling experience; or experience with the substance abuse population, gender-specific treatment, child and family counseling, or justice-involved population in a perinatal setting. Knowledge of SUD in adults. Skill in assessment, case management, advocacy, or individual and group therapy.
