Practice Care Manager Fayetteville, North Carolina, United States Patient Care Manager | Full-Time | North Carolina Cape Fear Family Medicine in Fayetteville, NC is searching for an experienced Patient Care Manager to join our staff. We are seeking a full-time Patient Care Manager that displays excellent teamwork, accountability, and will successfully function within a high-performing organization to join our team. Previous care management experience is requested and experience with the below would be a plus: Work in concert with the Physician and/or Provider to coordinate a full continuum of health care services considering the patient's unique social and cultural dynamics Assist patients in addressing concerns as needed through referral for assessment, and communication with healthcare team Maintain appropriate patient documentation in the EHR and care management software Develop and implement individualized care management plans for identified patients Provide patient education Connect patient with social and community support for continuity of care Educate patients and families on the importance of care management and the proper method to access care within the medical home environment Ensure follow-up with hospital discharge instructions for high risk, high acuity, high-cost recipients; ensure continuity of care Assist providers with coordination of services for high risk, high acuity, high-cost recipients by implementing Evidence Based Practices Advocate for patients to receive services that will improve their health condition Responsible for maintaining patient and family confidentiality in accordance with HIPAA Other job duties as required
