Role:
The AFL Provider plays a critical role in delivering high-quality health care and social assistance services to individuals requiring specialized support. This position focuses on assessing, planning, and implementing care strategies tailored to the unique needs of each client, ensuring their well-being and promoting independence. The provider collaborates closely with multidisciplinary teams, including medical professionals, social workers, and family members, to coordinate comprehensive care plans. Through continuous monitoring and evaluation, the AFL Provider ensures that care interventions are effective and adjusted as necessary to meet evolving client needs. Ultimately, this role contributes significantly to improving the quality of life for clients within the health care and social assistance sector across the United States.Minimum Qualifications:
Valid certification or licensure relevant to health care provision in the United States (e.g., CNA, LPN, or equivalent). Demonstrated experience in providing direct care or support services within a health care or social assistance setting. Strong understanding of client assessment, care planning, and documentation procedures. Ability to comply with all applicable health, safety, and privacy regulations. Excellent communication skills to effectively interact with clients, families, and multidisciplinary teams.Preferred Qualifications:
Experience working with diverse populations, including individuals with disabilities or chronic health conditions. Additional certifications such as CPR, First Aid, or specialized training in behavioral health. Familiarity with electronic health record (EHR) systems and data management tools. Background in social work, nursing, or related health care disciplines. Proven ability to manage complex cases and coordinate care across multiple service providers.Responsibilities:
Conduct thorough assessments of clients' physical, emotional, and social needs to develop individualized care plans. Deliver direct care and support services in accordance with established care plans and regulatory standards. Collaborate with healthcare professionals and social service agencies to coordinate and optimize client care. Maintain accurate and detailed documentation of client progress, interventions, and any changes in condition. Provide education and support to clients and their families to promote health, safety, and independence.Skills:
