Care Coordinator - Golden Harmony Home Care Golden Harmony Home Care, a locally owned and operated agency founded by Jen Stengl, was born from a deep, personal understanding of care and compassion. Key Responsibilities As a Care Coordinator, you will play a critical role in leading and supporting a team of caregivers and companions, ensuring that our clients receive the highest level of care.
Your responsibilities will include:
Leading and supporting a team of caregivers and companions, fostering a collaborative and compassionate environment. Overseeing the interviewing, hiring, and onboarding process for caregivers and companions, ensuring that we maintain a team of highly qualified professionals. Ensuring client satisfaction by maintaining high Net Promoter Scores and addressing any concerns promptly and effectively. Providing regular, constructive feedback to caregivers and companions, helping them grow and excel in their roles. Holding caregivers and companions accountable to high standards and company goals, ensuring consistency in care delivery. Managing the client database, ensuring that all records are current and accurate, including care plans, demographic information, and schedules. Overseeing staffing and scheduling to ensure that clients receive care from consistent caregivers or companions. Maintaining compliance with Department of Health and Human Services regulations for home care. Proactively working to increase client billable hours through effective care management. Conducting client visits and follow-up calls to ensure ongoing satisfaction and address any changes in care needs. Participating in the on-call rotation to provide support as needed. Why You'll Be a
Great Fit Preferred :
Bachelor's degree in Social Work, Geriatric Care Management, or a related field.
Preferred :
Two to three years of experience in case management or a related field, particularly in geriatric care.
Preferred :
Two to three years of experience in managing a team, with a proven ability to provide feedback and hold team members accountable. Demonstrated success in building strong relationships that contribute to achieving organizational and revenue goals. Strong organizational skills with the ability to manage multiple priorities and deadlines. Excellent communication and interpersonal skills, with a focus on active listening and effective problem-solving. Proficiency in using online scheduling systems and maintaining accurate records. Qualifications & Requirements Minimum of two years of experience in a similar role within home care or geriatric care management. Strong organizational and communication skills, with a proven ability to build relationships that drive organizational success.
