Carolina Outreach is a group of highly trained behavioral health professionals dedicated to promoting the emotional and physical well-being of our community in a safe and respectful environment. We provide a comprehensive and integrative array of behavioral health services and supports for people of all ages with varying levels of need utilizing an equitable and inclusive approach. About the Role The Behavioral Health and Intellectual/Developmental Disability (I/DD) Tailored Care Management (TCM) model reflects the goal of whole-person care management in NC Medicaid Managed Care. Provider-based care management promotes integrated care and offers beneficiaries a choice in how they receive care management. Develop care management comprehensive assessments and care plans/individual support plans with beneficiaries Coordinate/refer/monitor all services (medical, pharmacy, behavioral health, waiver services, food, housing, transportation, community resource supports) Support beneficiaries in a crisis (with planning supports) Arrange for annual physicals Innovations and TBI waiver care coordination (if applicable) Convene and consult with a multidisciplinary care team This description outlines the general scope and level of work for this role. Compensation:
$23.00 - $29.53 Placement within the range is based on licensure level, education, and experience . Qualifications:
A bachelor's degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area, or licensure as a registered nurse (RN) Two years of experience working directly with individuals with behavioral health conditions (if serving members with behavioral health needs) or with I/DD or a TBI condition(s) and For Care managers serving members with LTSS needs: two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience, in addition to the requirements cited above.
