Job Description

Position Summary The Collaborative Care Management (CoCM) Care Manager supports the delivery of integrated behavioral health services through population-based care management, psychiatric collaboration, and interdisciplinary care coordination. Working from a centralized office, the Care Manager partners with consulting psychiatrists, treating practitioners, nursing staff, and facility clinical teams to monitor patient progress, facilitate treatment planning, and support behavioral health outcomes. This role utilizes evidence-based Collaborative Care Management principles, including registry-based tracking, measurement-based care, psychiatric case review, and ongoing treatment monitoring. Direct patient interaction may occur as clinically indicated or upon patient request but is not the primary function of the position. Essential Responsibilities Clinical Monitoring and Assessment Registry Management Psychiatric Collaboration Care Coordination Documentation and Compliance Qualifications Required BA in Psychology, social work or other qualified behavioral health professional as permitted by applicable regulations. Experience working with behavioral health, psychiatric, geriatric, or chronic care populations. Strong clinical documentation and chart review skills. Knowledge of psychiatric diagnoses, psychotropic medications, and behavioral health treatment approaches. Ability to work collaboratively within interdisciplinary healthcare teams.
