Posting Summary Rutgers, The State University of New Jersey, is seeking a Nursing Care Coordinator in the Department of Medicine with the New Jersey Medical School (NJMS). In accordance with professional nursing standards, skillfully administers total patient care based on the individual needs of persons with tuberculosis and their families. This position interacts in a collaborative fashion, with the Lattimore Clinic Medical Director, the Nurse Manager, other nursing staff, and field staff to participate in TB training programs and research with the Global Tuberculosis Institute and with the NJ Department of Health. Among the key duties of this position are the following: Assesses the health status of assigned patients as outlined in the nursing standards. Ensures application of the core functions of public health related to tuberculosis care, through the use of the nursing process with the integration of nurse case management throughout the life-span. Plans, implements and evaluates individualized care based on patient assessment and medical plan of care. Provides direct patient care or direction and supervision to other health team members to ensure proper implementation of plan of care. Teaches patient/significant others about health/illnesses state and plan of care. Ensures that patient/significant others are prepared for discharge and health maintenance in the home setting.

Position Pension Eligibility:
ABP Minimum Education and Experience Registered Nurse Licensure in the State of New Jersey is required. Must maintain a license in good standing. At least three (3) years of outpatient experience. At least one (1) year of clinical nursing experience with familiarity in the use of electronic medical records (EMR). Certifications/Licenses New Jersey Registered Professional Nurse License and Registration; ACLS and

Effective oral and written communication skills are required. Ability to function in a team environment is required. Basic word processing and computer skills are required. Preferred Qualifications Bachelor of Science in Nursing (BSN) and is preferred. Physical Demands and Work Environment

Position requires standing 2/3 of the time, walking 2/3 of the time, requires sitting 1/3 of the time, use of hands to finger, handle or feel 2/3 of the time, reach with hands and arms, 1/3 of the time, stoop, kneel, crouch, or crawl 1/3 of the time, talk or hear over 2/3 of the time. Ability to lift and move patients with/without reasonable accommodations.
WORK ENVIRONMENT
Moderate noise (examples: business office with computers and printers, light traffic). Use of personal protective equipment including an N-95 respirator. May be required to accompany a field staff member for patient assessment outside of the clinic.
