Member Care Coordinator | Fieldwork | Raton, NM | Contract to Hire Walker Healthforce is seeking a Member Care Coordinator to conduct home health assessments, support member care plan engagement, and provide non-clinical support to a Medical Management team. This is a Contract opportunity.

Monday-Friday, 40 hours; 25% travel time
Worker Type:
W2 Core Requirements Bachelor of Social Work or Psychology;
OR LVN/LPN
with 1 year of managed care systems experience; OR RN; OR 3 years of care coordination for a state managed or waiver program; OR 3 years of managed care systems experience. Experience coordinating member medical-related needs, providing member assistance, and analyzing member needs. Knowledge of medical terminology and proficiency with Microsoft Office applications. Strong customer service, verbal and written communication, and member coaching skills, including motivational interviewing. Current state driver's license, transportation, applicable insurance, and ability and willingness to travel. We Consider It A Bonus If You Also Have Job Responsibilities Conduct home health assessments and update required systems. Perform member outreach, follow-up, and care plan goal check-ins. Educate members on health care programs and encourage care plan adherence. Coordinate appointments and additional member services, including transportation. Complete data entry, correspondence, research, and member/provider inquiry resolution. Provide non-clinical case support to the clinical team. Maintain confidentiality and comply with HIPAA and applicable policies.
