As the Health Home Care Coordinator Plus, you will play a pivotal role in individuals' lives, making a significant impact through: Accept referrals from supervisor and attempt to make contact with referrals within program guideline timeframes Gather input from providers to complete a comprehensive assessment Advocate with appropriate agencies for services and assist members in the scheduling and coordination of services Conduct home visits, as appropriate, and monitor member health status, access to services and quality of received services Document updates to care plan and assessment, including barriers and service access Research and obtain current information on targeted chronic illnesses

Monday - Friday 8:30-4:30

Qualifications:
