Job Description

Hiring Range:
$19.64
  • 26.28 hourly Below are the shift differentials based on eligible shifts worked: Weekday evening shift
  • additional 10% Weekday night shift
  • additional 15% Weekend day shift
  • additional 10% Weekend evening & night shifts
  • additional 21% The actual compensation for this position will be determined based on experience and other factors permitted by law.
Responsibilities:
Facilitates collaborative relationships with intra-hospital departments as well as community-based care/supports to ensure successful patient transitions in care. Develops, plans and conducts patient/family/care team meetings. Meet with patients ongoing care needs during the hospital stay, during transition/discharge, and post-acute care setting.
Additional Responsibilities:
Other duties as assigned.
Education, Training & Experience:
High school diploma or GED required 3 years of acute care experience, experience can include nursing. Certifications /
Licensure :
Case management certification preferred not required

Must be able to demonstrate effective and appropriate written and oral communication skills. Must be able to speak, read, write and follow instructions in English.
