Job Description

Company Overview Building Blocks is a multi-service developmental and educational organization established in 2004. Our mission is to help children and adults achieve their greatest potential through high-quality care provided by our dedicated team of therapy and educational professionals. Job Description The Adult Care Manager conducts assessments and creates a care plan with support of a care team to assure the client receives quality services to maintain optimum healthcare needs. The primary goal of an Adult Care Manager is to aid in the maintenance of the individual's healthcare and social needs by advocating on their behalf, assist in scheduling and time management and linking the patient to services.
Essential Functions:
Outreach and engagement to formally enroll referred individuals into the care management program.
Individuals may include:
adults (ages 18+) with severe mental illness, recently released from varying rehabilitation programs, mental health facilities, incarceration, etc. Conduct assessments, evaluate needs, establish and maintain care plans and maintain referrals to services for enrollees. Assures supports are in place inclusive of peer and family contacts. Develops interim care plan based on preliminary clinical information. Collaborate with all service providers and establish a team communication plan. Monitor goals on a continuing basis and assure team communication Consult with family members and social supports to maintain support consistency. Advocate for additional services and link as appropriate. Ensure maintenance of linkages. Maintains current care management documentation and information regarding care management activities within the required health IT system(s). Ensure compliance to all government and agency regulations/policies. Adhere to required program and internal deadlines. Perform all other duties relevant to the position as requested.
Position Requirements:
Successful clearance through a required background check, Medicaid Staff Exclusion List, and Child Abuse Registry Applicable Department of Health trainings/requirements Applicable vendor and other government trainings/requirements
Education & Experience:
Associate's Degree (Bachelors Degree preferred) in Psychology, Sociology, Human Services or related field 1-2 years experience in providing Care Management Services or related experience (preferred, but not required) Basic computer skills (Google Suite & Adobe Acrobat) Travel is required, must have a valid NYS drivers license.
