Job Summary:
A Social Services Coordinator is responsible to provide the hospital, under the general supervision of the Case Manager and the Director of Case Management, comprehensive and professional transfer planning services, resource information, and general assistance as needed. Provides a leadership role in the discharge planning component to patient care, serving as a resource for the identification and implementation of post hospital care and service delivery systems. Protects the rights, troubleshoots the concerns, and promotes the interest of patients in the medical center. Complaint Management to reduce liability is a key component of the Risk Management program.
Qualifications:
EDUCATION
Associates degree in social work or a related field required
LICENSES AND CERTIFICATIONS
BLS certification required TRAINING and
EXPERIENCE
Minimum two years of related experience in a healthcare setting with knowledge of community resources Ability to assess data reflective of the patient's status and interpret relative to age specific needs Ability to be self-motivated, independent, professional, creative and dependable Ability to work with others as a team to ensure quality patient care and efficiency Basic computer knowledge Maintaining confidentiality of all patient information Maintaining quality assurance and compliance with all regulatory agencies Ability to implement and support procedures and programs pertaining to patient safety and security Exhibit proficient communication skills and ability to deal efficiently and effectively with a variety of personalities Ability to sustain attention to detail and accuracy as it pertains to the monitoring and documentation of information
