Crouse Health offers a full range of general and specialty care, inpatient and outpatient services, as well as community health education and outreach programs. Crouse's Care Coordination team is hiring a Care Coordination Assistant .
Pay Range:
$19.50 - 30.93 / hourly
Schedule:
Full-Time, Monday-Friday, 8:00am-4:30pm .
Care Coordination Assistant Responsibilities:
Responsible for collaboration with the Care Coordination staff to assist with meeting the department and organizational goals. Delivery and tracking of the "Important Message from Medicare" to Medicare beneficiaries who are in the hospital as inpatient status and documenting that this was done. Responsible for daily maintenance and updating of the Care Coordination databases. Navihealth referrals. EMR documentation. Communication with community health care providers as indicated for patient care transition. Provide back-up to the Care Coordination administrative services and duties. Assist the Director and Care Coordination staff with other tasks as needed.
Care Coordination Assistant Requirements:
High School Diploma or GED required. Associate's Degree preferred Two (2) to three (3) years' experience in Health Information Management (HIM), Business Office/Billing, Patient Access, or Case Management is required. Must have the ability to manage time effectively and communicate in a professional manner. Must be able to use Microsoft Office programs including Outlook and a variety of department specific databases and systems (Epic). Must be able to multi-task and remain flexible in a continuously changing environment.
