Peer Outreach Specialist Position Summary:
The Peer Outreach Specialist will provide support to individuals transitioning from skilled nursing facilities and/or intermediate care facilities (ICFs) to independent living in the community.
Job Responsibilities:
Provide one-on-one, face-to-face peer support for assigned residents in the skilled nursing facilities and/or the intermediate care facilities (ICFs); Provide assistance with the resident's discharge planning process for transition back to the community. May attend discharge and transition planning meetings on behalf of the resident. Collaborate with Peer Supervisor to identify and address any barriers, concerns, or additional counsel required. Accurately document all peer visits, in a timely manner, in the NYAIL Database; and Attend required training courses, meetings and other agency-related activities. Perform other program-related duties assigned.

High-level confidentiality is required for access to all organization records.

Office functions are performed while maintaining a safe work environment.
Qualifications:
Experience as a Peer or comparable peer employment Must have been a recipient of long-term care services and/or receiving services at home or in the community and must be comfortable in referring to that experience when delivering trainings Demonstrated ability to engage with diverse groups Ability to collaborate with other trainers and identified stakeholders Must have access to reliable transportation
Education and Experience:
High School Diploma/GED preferred Physical Requirements Essential for Safe and Effective Job Performance Ability to successfully remain sitting or working at a desk for prolonged periods. Vision sufficient to maintain a New York State Driver's License and constant use of a computer. Speech and hearing sufficient for normal communication. Ability to successfully work on a computer as a significant responsibility. Full range of motion to move safely in the office and outside locations which may not have accommodation. May be required to put away office supplies and transport event materials. Must be able to lift, push, pull, walk, stoop, reach in all directions to move freely and quickly within any area. Must be able to lift or move items necessary for events and other related activity. Finger manipulation and hand coordination for constant computer use. Ability to travel in your own vehicle to all job-related out-of-office functions or meetings as required. Ability to report on time and remain for overtime as needed.
Commitment Requirement:
Complete the mandatory eight-hour training session within two months of employment. The time commitment for each session includes preparation, travel, one or two hours of meeting, and post reporting. Must be willing to participate in occasional Peer training opportunities and meetings Work location: Syracuse - Onondaga County Hours and days: Peers will be utilized as requests for peer services are received
Compensation:
$17.00 -$18.00 per hour Non -Exempt This is a Part Time position
