This role is essential for ensuring that our services meet the highest standards of quality and compliance while providing compassionate care to our clients. The ideal candidate will have a strong background in home health care and/or long-term care facilities. Duties Develop and implement effective care plans tailored to individual client needs, including assessments of Activities of Daily Living (ADLs). Collaborate with healthcare professionals to coordinate outpatient services and acute care as needed. Ensure proper documentation and record-keeping for all patient interactions and care plans. Monitor the quality of care provided by staff, addressing any issues or concerns promptly. Maintain relationships with clients, families, and community resources to enhance service delivery. Experience Proven experience in PT, OT, or ST and long-term care setting. Knowledge of outpatient services, acute care settings, discharge planning processes, and developing comprehensive care plans is essential. Strong leadership skills with the ability to motivate. Excellent communication skills to interact positively with clients, families, and healthcare professionals.
Pay:
$55.00 per hour
