You are a caring professional who is dedicated to working with clients through all walks of life that includes but is not limited to homelessness, substance use, primary mental health diagnosis and/or co-occurring disorders, incarcerated population. While being housed at our inpatient residential facility, you will make a difference in your community by providing care coordination services through direct contact with clients, families and essential others in their natural environment. Under the guidance of the Crisis Manager, you will serve as a client advocate, acting in their best interest at all times. You will attend treatment plan development and review meetings. You will also coordinate and monitor appropriateness of all services, examine and recommend new programs, and provide assistance in crisis situations to stabilize your clients. You will ensure adequate client care, provide consultative services and/or promote the coordination and development of mental health and/or substance abuse services within the community. Record client progress, monitor relapse plan and maintain necessary records and files. You will provide support, resources and linkage to services for your clients by maintaining a relationship with other agencies or external service providers as needed.
