This is a hands-on role combining client reassessment, caregiver supervision, orientation, documentation, and team coordination . The Care Coordinator works closely with our Care Manager and Scheduling team and spends time both in the office and visiting clients in the field.
The goal is simple:
know what is happening with our clients and caregivers, identify problems early, and make sure appropriate action is taken. Primary Responsibilities Manage ongoing coordination of established clients after initial onboarding and stabilization. Complete scheduled and incident-driven client reassessments. Conduct caregiver supervisory visits and observe performance in the home. Identify and document changes in client needs or condition. Coach caregivers and document performance concerns when necessary. Escalate at-risk or unstable clients to the Care Manager. Participate in and lead caregiver orientation twice weekly. Work closely with Care Management and Scheduling on client needs and caregiver assignments. Maintain timely, accurate documentation and follow-up. Workload Structure This position follows an 80% planned / 20% responsive model . The successful candidate must be able to adjust priorities without losing track of existing responsibilities. Qualifications Strong organization, time management, and follow-through Ability to independently manage multiple responsibilities Strong written and verbal communication Comfortable providing feedback and holding others accountable Sound judgment and ability to recognize when concerns should be escalated Comfortable working independently in client homes and in an office environment Reliable transportation for local client visits Basic computer and electronic documentation skills Home care, healthcare, senior services, caregiving, or supervisory experience preferred Preferred Traits We are looking for someone who: Keeps track of multiple moving pieces without constant reminders Notices problems and follows them through to resolution Can distinguish what is urgent from what can wait Communicates concerns early Is comfortable having professional but difficult conversations Documents consistently and closes the loop Works independently while remaining connected to the team Can adapt when the day's plan changes Care experience is valuable, but organization, judgment, communication, and follow-through are the most important qualities for success in this position.
