Job Description

Job Summary:
Works closely with the Yellowhawk provider team and responsible for coordinating both internal and external referral care components to provide the best patient care possible. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.
Essential Job Functions:
Works closely with assigned provider/s, nursing staff and patient panel on a daily basis to accommodate the patients outside scheduling needs and processing referrals. Works with outside medical providers and agencies in coordinating patient care, ensuring patients are provided quality health care while utilizing alternate resources to maximum capacity. Develops referral systems with community agencies as necessary. Works with appropriate Yellowhawk staff and outside vendors to ensure referrals get completed accurately and timely, while following guidance of the Yellowhawk Compliance team. Maintain highest level of confidentiality. Works with patients to ensure they receive appropriate Yellowhawk services. Understands and complies with various Yellowhawk policies and procedures. Works with the outside vendors and community to educate patient population regarding alternate resources, making sure the patient understands the processes required for referral/consult approval. Work well with others and foster a team environment to support all Yellowhawk departments with regard to the referral processes. Work toward cross-training as coverage when in need of the Eligibility Coordinator. Other duties assigned.
Minimum Qualifications:
High School Diploma or equivalent One (1) year of successful experience of administrative/clerical in an office setting. Demonstrated experience with Microsoft Office programs and/or other relevant software to meet and/or other relevant software to meet the needs of the various programs. Demonstrated high level organization and planning skills. Demonstrated knowledge of standard office administrative practices and procedures. Demonstrated ability to manage multiple tasks at once. Must submit to and pass a reference and criminal background check. Must submit to and pass pre-employment drug and alcohol screening.
Preferred Qualifications:
Working knowledge of medical benefit payments programs such as Medicare, Medicaid, private insurance, etc. Experience in a medical/clinical office. Good understanding of medical terminology and familiarity of medical acronyms. Experience with Electronic Health Records/RPMS. Experience in Tribal healthcare system.
