Job Description

Job Summary:
The RN will provide comprehensive preventative and therapeutic nursing care in an ambulatory health care setting. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.
Essential Job Functions:
1. Promotes and assists in restoring the patients' health by completing the nursing process, including triage, appropriate medical testing and test interpretation, case management, consulting with providers, and related services and activities. 2. Assists in the proper use and maintenance of technical medical equipment and patient examination rooms. 3. Collaborates with physicians and multidisciplinary team members and carries out duties as requested. 4. Provides physical and psychological support to patients, friends, and families; supervises assigned team members. 5. Fulfills crucial functions to ensure quality patient care. May include care for patients, filling out paperwork, monitoring the waiting room, assisting with referrals and transferring records and reports. 6. Ensures patient records are properly, accurately and timely documented using selected computer hardware and software. 7. Demonstrates knowledge and skills necessary to offer care appropriate to a wide population of patients and have knowledge of community resources. 8. The Case Manager
  • RN has long-term, regular caseloads and provides supervision of ancillary staff (RNs, LPNs, CNAs, PCAs) to provide a holistic approach to care which includes educating and advocating for client's individual needs 9.
Acts as a resource to patients and their families by providing a variety of educational information and support. 10. Ability to safely administer viral/bacterial swab testing and vaccine administration. 11. Performs duties in specialty clinics, as assigned. 12. Other duties as assigned.
Minimum Qualifications:
1. Graduation from an accredited technical school of nursing; ADN or BSN required. 2. Current license as a Registered Nurse in State of Oregon. 3. Current BLS certification. 4. Knowledge of HIPAA regulations. 5. Proficient in Microsoft Word and Excel. 6. Experience using electronic medical records. 7. Ability to multi-task while maintaining high level of organization and attention to detail. 8. Excellent telephone, customer service and communication skills. 9. Ability to work collaboratively in a team-based setting as an effective team player. 10. Represents Yellowhawk with a respected and viable presence in the community. 11. Must submit to and pass a reference and criminal background check. 12. Must submit to and pass a pre-employment drug and alcohol screening.
Preferred Qualifications:
1. Two (2) years of ambulatory care nursing experience, nursing or emergency/urgent care experience preferred. 2. One (1) year clinical, case management, or utilization review experience in an acute care setting. 3. Prior experience with Resource and Patient Management System (RPMS) Practice Management software is preferred.
