$29 - $33 Job description: Overview We are seeking a dedicated and compassionate Qualified Professional (QP) to join our dynamic team. This vital role involves monitoring, recording, and supervising personal care services performed by the Caregiver to their respective clients. The QP does this through periodic visits to the client's home (or via phone for every other visit) in a timely manner according to DHS standards. The visits include client care plan development, monitoring, and training of the Caregivers based on the care plan. Supervision of the services and also includes evaluating Caregivers, once trained, as to whether they can perform the client's cares properly and are able to communicate and document changes in the client's condition. A QP may also spend time with the client helping them maintain their services through submitting documentation to their county of residence or to the Department of Human Services. Performing these tasks according to Department of Human Services' guidelines will ensure recipients of services are receiving them appropriately and to their expectations. Duties Develop, implement, and review individualized care plans tailored to each client's needs, including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support The QP must be able to submit documentation in a timely manner Ability to work with little direct supervision and make appropriate judgments Demonstrate dependability, tact, and ability to follow orders Ability to communicate effectively with the client and Shifa Home HealthCare, Inc. staff, respond appropriately to client needs, and report changes in the client's condition to the Shifa Home HealthCare, Inc. Ability to pass a MN Department of Human Services background check Ability to join a once-a-month team meeting In order to apply for this position, you must be one of the following: A Registered Nurse must be licensed with the Minnesota Board of Nursing and maintain an active status. Proof of active nursing status must be provided to AHH, Inc. A Licensed Social Worker A Mental Health Professional (defined as one of the following): a licensed independent clinical social worker or a psychologist licensed by the Board of Psychology or a physician licensed under chapter 147 if the physician is: (i) certified by the American Board of Psychiatry and Neurology; (ii) certified by the American Osteopathic Board of Neurology and Psychiatry; or (iii) eligible for board certification in psychiatry or a licensed marriage and family therapist or a licensed professional clinical counselor. A Qualified Designated Coordinator (must have at a minimum): A baccalaureate degree in the field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older or An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older or A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older. • Monday to
