Description Care Coordinator - Birth to Three Program Full-Time | Hybrid | Waukesha, WI Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a full-time Care Coordinator to join our Birth to Three Program , a federally mandated Early Intervention program supporting children ages 0-3 with developmental delays or disabilities and their families. This role focuses on strengthening families, enhancing child development, and coordinating services using a family-centered, trauma-informed approach. This is a full time, benefit eligible role working Mondays-Fridays from 8am-4/4:30pm. After a training period, this role will have remote flexibility when not at home visits with clients, but the team does meet at the Waukesha office every other Tuesday for mandatory staff meetings. What You'll Do As a Care Coordinator, you will partner closely with families, service providers, and community resources to support young children's development.
