Full-Time (37.5 hours/week)
Pay Range:

What You'll Do Conduct assessments to determine program eligibility and care needs Develop and manage individualized care plans Monitor service delivery and adjust plans as needed Coordinate with providers, caregivers, and internal teams Advocate for client safety, independence, and well-being Maintain accurate documentation and meet program standards Why It Matters Your work helps older adults remain in their homes with dignity - something 9 out of 10 say they want.
