Right at Home is seeking a compassionate, organized, and motivated Care Coordinator to join our growing team. This role offers a unique combination of client care, caregiver training, leadership, and care coordination. As a Care Coordinator, you'll work closely with clients and their families while providing ongoing support, education, and guidance to caregivers. This is an excellent opportunity for someone who enjoys both hands-on care and leadership and wants to grow professionally within a supportive, team-oriented environment. While some remote work is available, this position is primarily field-based and requires regular in-person visits with clients and caregivers. Participation in the on-call rotation, including one weekend per month, is required. Responsibilities Conduct new client assessments Complete initial client visits and help ensure a smooth transition into services Conduct quarterly supervisory visits for clients Develop, maintain, and update individualized care plans Provide hands-on training and support to caregivers in the field Lead and support a small team of
Care Leads Qualifications Required:
Associate degree or higher Previous direct caregiving experience Strong organizational and communication skills Ability to prioritize tasks and manage multiple responsibilities Valid driver's license and car insurancePreferred:
Three years of management or supervisory experience Experience training, coaching, or mentoring caregiversThe Ideal Candidate Is:
