Job Description

Clearix RCM is seeking a detail-oriented and empathetic CCM Coordinator to manage the delivery of non-face-to-face care coordination services for patients with two or more chronic conditions. The primary goal is to improve patient outcomes, reduce hospitalizations, and ensure all CMS requirements for CCM billing are met and documented with precision. Key Responsibilities 1. Care Coordination & Patient Outreach Conduct monthly telephonic check-ins with enrolled patients (minimum 20 minutes of qualifying care per month). Act as the primary point of contact for patients, helping them navigate their care plans and healthcare providers. Coordinate transitions of care, including follow-ups after ER visits or hospital discharges. 2. Care Plan Management Develop, implement, and update Comprehensive Care Plans in the EHR. Monitor patient adherence to medications, diet, and specialist appointments. Identify potential health gaps and escalate clinical concerns to the appropriate provider or NP. 3. Documentation & Compliance Maintain meticulous logs of all "clinical staff time" spent on patient-related activities to support CPT 99490 and related codes. Ensure patient consent is properly documented and updated annually. Ensure all interactions meet HIPAA privacy and security standards. 4. Technical Navigation Should have past experience with ecw EMR Manage tasks within a HIPAA-compliant VoIP system and secure messaging portals.
Qualifications & Skills Education:
Medical Assistant (CMA/RMA), LPN, or RN license preferred (required depending on state/contract requirements).
Experience:
2+ years in healthcare, ideally in primary care, internal medicine, or geriatric care management.
Technical Proficiency:
• Prior experience with eClinicalWorks (eCW) or similar EHRs is highly preferred. Comfortable using VoIP phone systems and remote collaboration tools.
Knowledge:
Strong understanding of chronic conditions (Diabetes, Hypertension, COPD, etc.) and Medicare CCM billing guidelines.
Communication:
Exceptional verbal communication skills with a "patient-first" bedside manner over the phone.
Bilingual:
Fluency in Spanish is a plus.
Core Competencies Time Management:
Ability to manage a high volume of patients while meeting strict monthly minute requirements.
Critical Thinking:
Ability to spot "red flags" in a patient's self-reported health data and act quickly.
Self-Driven:
Capability to work independently in a remote environment while maintaining high productivity.
