This is a structured clinical coordination role with measurable daily and monthly performance expectations. The Care Coordinator is responsible for ensuring each assigned patient receives required monthly care management services and that all documentation meets Medicare compliance standards. This is not a passive administrative position. This role requires focus, efficiency, and strong communication skills. Position Overview The CCM Care Coordinator will manage an assigned panel of patients with multiple chronic conditions. Core Responsibilities Patient Management and Outreach Conduct structured monthly check-in calls with assigned CCM patients Assess symptoms, medication adherence, barriers to care, and goal progress Monitor chronic conditions including but not limited to cardiovascular disease, diabetes, COPD, sleep apnea, hyperlipidemia, hypertension, and behavioral health conditions Identify changes in condition and escalate appropriately Care Plan Management Create and update individualized care plans in ThoroughCare Ensure care plans reflect current diagnoses, goals, and interventions Document measurable patient goals and follow-up actions Track progress and adjust plans as needed Care Coordination Coordinate with primary care providers and specialists Follow up on labs, imaging, and referrals Assist with appointment scheduling when needed Communicate clinically relevant updates to supervising provider Qualifications Medical Assistant certification or equivalent clinical experience preferred Prior experience in primary care, care coordination, or chronic disease management strongly preferred Comfortable managing high patient volume and structured daily call schedules Strong documentation skills and attention to detail Ability to communicate clearly and confidently with patients Organized, self-directed, and able to work independently Knowledge of HIPAA and Medicare compliance standards
