This vital role involves coordinating patient care, managing communication between patients and healthcare providers, and ensuring seamless delivery of services across various clinical settings. The ideal candidate will possess a strong understanding of medical terminology, hospital workflows, and patient-centered care principles. Responsibilities Act as the primary point of contact for patients, families, and healthcare providers, ensuring clear communication and timely information exchange Coordinate patient intake, discharge planning, and follow-up care to streamline clinical workflows Assist with utilization review and utilization management processes to optimize resource use and ensure compliance with healthcare standards Support case management activities by tracking patient progress, updating electronic health records (EHR) systems, and facilitating interdisciplinary collaboration Maintain thorough documentation of patient interactions, medical records, CPT coding, ICD-9/ICD-10 coding, and other relevant clinical data Collaborate with social work teams, behavioral health specialists, and hospice care providers to address complex patient needs Ensure adherence to HIPAA regulations by safeguarding patient information during all interactions and documentation processes Experience Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and discharge planning procedures Experience working with EMR (Electronic Medical Records) or EHR systems is essential Familiarity with managed care processes, utilization review, and case management practices highly desirable Excellent communication skills with the ability to handle intake assessments and coordinate multidisciplinary teams effectively Ability to thrive in fast-paced environments requiring attention to detail and critical thinking skills Join us in delivering exceptional patient service through proactive coordination and compassionate support.
