M-F 8:00 AM-4:30PM (Potential for participation in On Call Rotation) Description of the
Role:
As a Client Care Coordinator, you'll be at the heart of our care delivery team. You'll oversee caregivers, scheduling, client care coordination, client assessments, care plans, starts of care, call offs, and some office administrative duties. This is a hybrid role meaning there is office work and work in the field. You'll work closely with the office team to ensure caregivers are effectively matched with clients, mentored with compassion, and equipped to provide outstanding care. The ideal candidate for this role is someone with a people-first work style, excellent organization and time management skills, and acommitment to ensuring the highest care delivery standards.
Job Duties:
Efficiently schedule caregivers with clients, ensuring compatibility of skills and care needs to ensure client satisfaction Communicate regularly with the team, client's family or representatives regarding scheduling updates Utilize effective staffing skills and regularly monitor staffing statistics to strategically increase client hours and avoid overtime Ensure proper pay rates, billing codes, and shift accuracy in Wellsky Conduct supervisory site visits to provide coaching/counseling to caregivers Create individualized care plans for clients and update them as needed Conduct in home client assessments Conducrt in home safety assessments Provide training to caregivers at the start of care Ensure caregivers are compliant with their continued education/training programs Collaborate closely with the team, be available for starts of care when needed and help to ensure there is a sufficient caregiverpool Travel within the Bellevue/Redmond/Kirkland/Mercer Island and surrounding areas as needed or assigned Occasionally provide in-home support to clients, covering for caregivers who call-off, when necessary Potential to participate in regularly scheduled on-call rotation handling off-hours staffing needs
Qualifications:
2+ years of experience in home care, client management, or related role with transferrable skills preferred Valid Driver's License and vehicle insurance with clean DMV record required Current CNA or HCA Certification required Proficiency in using healthcare management software preferred or willingness to learn
