Discharge Planning Coordinator

This is a hybrid position where you will split your work time between the office, community, and home. What You Will Be DoingAs a Discharge Planning Coordinator, you will have the opportunity to work closely with community hospitals to complete a biopsychosocial assessment to determine eligibility for ongoing services and supports to promote successful discharge into the community. You will also be responsible for administrative case management functions and supporting team operations by maintaining accurate documentation and ensuring all records meet Home and Community Based Services (HCBS) requirements. You will be an active member of an internal committee which completes training for primary clinicians on the level of care tool utilized for individuals diagnosed with a mental illness. This position provides occasional back-up assistance for the ISK Hospital Liaison which will include, but is not limited to, performing length of stay reviews. In addition, this position also provides back-up to the Specialized Residential Team who works closely with primary clinicians providing utilization management to determine the appropriate placement level of care for individuals served. What We RequireA Master's degree licensed professional with a minimum of three years' experience in behavioral health care services working with adults is required. Experience working with individuals who have had a psychiatric admission or lived in a specialized residential setting and familiarity with HCBS rules and regulations preferred. You must be licensed in Michigan as one of the following: Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC). Limited licenses will be considered. A valid Michigan Driver's License is required. A strong understanding of the needs of persons with mental illness, intellectual/developmental disabilities, and substance use disorders is needed. You will understand how to implement a strong recovery philosophy and be able to collaborate and develop teams. Physical Requirements/Working Conditions:

Physical Efforts - Job demands include prolonged sitting and standing as appropriate. May occasionally require light lifting up to 25 pounds, stooping, kneeling, crouching, or bending as appropriate. Requires coordination of hands and/or eye/hand/foot. Working Conditions - Office environment with noise from computers, copy machine, and telephones. Use of computer for periods in excess of 30 minutes at a time. Possible eyestrain from extended periods of computer screen. May be exposed to bloodborne pathogens, infectious diseases, and parasites. Travel throughout the Kalamazoo area is required.
